Voice calls need guaranteed low latency and consistent bandwidth — you cannot send voice over a best-effort default bearer (QCI 9). When a VoLTE call is set up, the PCRF triggers a dedicated bearer with QCI 1: guaranteed bit rate (GBR), low latency (100 ms budget), low packet loss (10^-2). This bearer exists only for the duration of the call.
